How to fill the Upstox account closure form?

The Upstox demat account closure form is a simple 1-page form. Follow the below steps to download the form.

• Visit www.upstox.com and go to Support >> Download Forms
• Scroll to 'Account Modification Forms' section
• Download the 'Demat Account Closure' form
• Print it.

The Upstox account closure form has 4 parts:

1. At the top of the form, enter the date and select 'BO' in the Closure Initiated by row.
2. Fill account holder details like Client ID, name or names in case of joint accounts, address, city etc.
3. Fill in details like a reason for closing the account, the select status of Balance securities remaining in the account, DP ID of the demat account where you want existing securities to be transferred.
4. All account holders need to sign on the form.

The second part is to pick a choice for securities held in the demat account and finally the signature of all account holders.
