As you want to change your name from Laxmi D to Laxmi P, you need to submit the Change/Correction Request Form along with the required document.

You can get the PAN Card change request form the official website of NSDL PAN Service

The fee for making any change to your PAN Card to Rs 107.

You can track the status of you application online. It usually takes 15-20 days to get the new PAN card.
